This letter of agreement is a contract for my services.

A deposit of half your total is due to reserve your wedding date.

This Agreement by and between Reverend Ann Cruz (hereinafter called the Officiant) and the bride and groom (hereinafter called the Couple or Partner) agree as follows:

The Marriage License:

The license, if required, must be obtained in person by the couple at the appropriate County Clerk's office.

The marriage license must be presented to the officiant at the rehearsal or prior to the start of the ceremony if there is no rehearsal.

The officiant will mail the completed marriage license within 5 (five) business days after the completed ceremony to the issuing agency.

Payment of Fees:

A completed agreement form is required to reserve ceremony date and time in Officiant’s calendar.

50% of the total invoice is due immediately in order to reserve the wedding date.

All ceremony fee balances shall be paid in full 30 days prior to the day of the ceremony, and are non-refundable.

Methods of Payment:

Credit card (or debit card) payments are preferred.
However, the officiant accepts cash, personal check, cashier’s check, or money order.

Payments made by credit card are processed by Rev. Ann Cruz using the "Square" system.

Payments made by checks or money orders are to be made payable to “Rev. Ann Cruz” and mailed to 14900 Carpenter Rd., Three Oaks, MI 49128.

Late Start Weddings Due to Bride or Groom Issues:

If for ANY reason by no fault of the officiant, the ceremony starts 45 minutes or more after the scheduled start time, the officiant may, at her option, modify the contents of the ceremony/script for time, as needed.

Contract Termination:

Officiant reserves the right to cancel services to the couple if a safe environment is not provided and/or the officiant is under duress and/or threat from any person.


In general, fees paid are non-refundable.

The deposit is non-refundable, however, is transferable to another date and time.

The couple must request this at least one week before the original reserved date.

If paid in full, a refund (minus the deposit) will be issued if canceled prior to two weeks before the reserved date.

In the event that the Officiant is unable to perform the wedding ceremony due to illness, hospitalization, accidents, transportation breakdown or other unforeseeable causes, she will make every possible attempt to notify the Couple and arrange for a suitable substitute officiant.

The Couple hereby agrees that Officiant will NOT be held liable for any damages (including punitive) due to the non-performance of the ceremony or function.

The undersigned understands, agrees and consents to release and indemnify the Officiant from and against any and all liability, claims, damages, and actions arising directly or indirectly from her services.

This agreement and any attachments constitute the entire agreement between the parties and may not be modified except in writing signed by both parties.
No other representation or promises have been made except those that are set out in this agreement.

If any part of this agreement is adjudged to be invalid, illegal, or unenforceable, the remaining parts shall not be affected and shall remain in full force and effect.

IN WITNESS WHEREOF, the parties hereto agree to the terms and conditions as described above and have caused this Contract to be signed below.

The use of a typed signature constitutes your acceptance and agreement as if actually signed by you in writing.
